Tom Rifai, MD, FACP, DipABLM, is an Internal Medicine physician specializing in metabolic health, lifestyle medicine, nutrition, and transformational lifestyle change. With more than two decades of experience, Dr. Rifai helps patients improve weight, blood pressure, cholesterol, and blood sugar by using lifestyle as a central component of care, along with medications when clinically appropriate.

Dr. Rifai’s approach is grounded in both medical science and personal experience. His own recovery from binge eating disorder shaped his understanding of the behavioral and psychological challenges that can make lasting lifestyle change difficult. He combines evidence-based medicine with practical behavior-change strategies to help patients develop healthier habits that are realistic, sustainable, and suited to everyday life.

Throughout his career, Dr. Rifai has held leadership roles in metabolic health, nutrition, lifestyle medicine, and health coaching. He previously served as President of the National Board of Physician Nutrition Specialists and serves as Chair of the Nutrition Working Group for the Healthy Longevity Medicine Society. He is also President and CEO of Reality Meets Science® Inc.

At CoreMed Plus, Dr. Rifai serves as Medical Director of Metabolic Health & Weight Management, leading a personalized program that combines one-on-one physician care, nutrition and lifestyle coaching, telehealth, group support, and digital tools. His Flex5 Lifestyle® framework focuses on helping patients build sustainable skills around nutrition, physical activity, mindset, environment, and accountability.

Dr. Rifai’s philosophy centers on creating health plans that patients can realistically maintain, helping each person find the place where their health and happiness meet.

Dr. Mark Richter Bio

I grew up in Florida as the oldest of five sons. After earning my degree from the University of Florida, I pursued my medical education in the Dominican Republic. In 1988, I began my Family Practice Residency at the “old” Pontiac General Hospital. I have been providing medical care to the same community since 1991, first in Waterford and later at our current location in White Lake, where I continue to practice today. I take pride in being board certified by the American Academy of Family Physicians and remain committed to Primary Care. I am passionate about promoting a healthy lifestyle, including a proper “balanced” diet, exercise, exposure to healthy outdoor activities, and achieving work-life balance. I married my Medical School Sweetheart, Barbara, and we are fortunate to have two adult children and a granddaughter. We have been residents of Oakland County since 1988 and enjoy all that Michigan has to offer.
