About Carolyn Cianfarani, RD, CSG

Registered Dietitian & Care Manager

Carolyn Cianfarani, RD, CSG, is a Registered Dietitian and Board-Certified Specialist in Gerontological Nutrition with extensive experience in nutrition care, patient education, and long-term care. She is dedicated to helping patients improve their health and well-being through personalized nutrition guidance, education, and compassionate, patient-centered care.
Carolyn earned her degree in Dietetics from Wayne State University in 2015. Throughout her career, she has focused primarily on long-term care, where her passion for supporting older adults inspired her to pursue advanced specialty certification in gerontological nutrition.
Her professional experience spans a variety of healthcare settings, including hospitals, bariatric weight management clinics, oncology care, and community-based nutrition programs. She also worked with youth in Detroit through a Wayne State University nutrition program focused on motivational interviewing, helping individuals develop healthier behaviors through personalized guidance and support.
During her years in long-term care, Carolyn expanded her experience beyond nutrition by serving as a backup social worker. Working closely with an experienced mentor, she developed a deeper understanding of the emotional, social, and practical challenges patients face, strengthening her ability to provide comprehensive, compassionate care.
At CoreMed Plus, Carolyn serves in a dual role as Registered Dietitian and Care Manager, combining her expertise in nutrition with care coordination and patient support. Her approach emphasizes individualized care, practical nutrition strategies, and helping patients navigate their healthcare needs to achieve better health outcomes.
Carolyn believes that effective healthcare goes beyond treating medical conditions. By understanding each patient's unique needs, lifestyle, and challenges, she strives to empower individuals to make informed decisions, build healthier habits, and improve their overall quality of life.

Dr. Mark Richter Bio

I grew up in Florida as the oldest of five sons. After earning my degree from the University of Florida, I pursued my medical education in the Dominican Republic. In 1988, I began my Family Practice Residency at the “old” Pontiac General Hospital. I have been providing medical care to the same community since 1991, first in Waterford and later at our current location in White Lake, where I continue to practice today. I take pride in being board certified by the American Academy of Family Physicians and remain committed to Primary Care. I am passionate about promoting a healthy lifestyle, including a proper “balanced” diet, exercise, exposure to healthy outdoor activities, and achieving work-life balance. I married my Medical School Sweetheart, Barbara, and we are fortunate to have two adult children and a granddaughter. We have been residents of Oakland County since 1988 and enjoy all that Michigan has to offer.
