Chronic Diseases Management

Chronic diseases, or non-communicable diseases (NCDs), are long-term medical conditions requiring ongoing management and care. Examples of chronic diseases include diabetes, heart disease, cancer, and chronic obstructive pulmonary disease (COPD). According to the World Health Organization (WHO), chronic diseases account for 71% of all deaths globally and are the leading cause of death and disability worldwide.

Managing chronic diseases can be challenging and complex, as these conditions often require ongoing medical care, lifestyle modifications, and multiple medications. CoreMed Plus is a healthcare provider specializing in chronic disease management, offering comprehensive care to patients with a wide range of chronic conditions.

At CoreMed Plus, the focus is on providing personalized, patient-centered care that is tailored to each individual’s unique needs and circumstances. The healthcare team at CoreMed Plus includes physicians, nurses, pharmacists, and other healthcare professionals who work together to develop a customized treatment plan for each patient.

One of the key components of chronic disease management at CoreMed Plus is education. Patients are empowered with the knowledge and skills to manage their condition effectively, including information about their condition, medication management, and lifestyle modifications. Education is provided in various formats, including one-on-one consultations with healthcare professionals, group education sessions, and educational materials such as brochures and videos.

In addition to education, CoreMed Plus offers a wide range of services to help patients manage their chronic conditions. These services may include regular check-ups, laboratory testing, medication management, and referrals to specialists as needed. The healthcare team at CoreMed Plus works closely with each patient to monitor their condition and adjust their treatment plan as needed to ensure the best possible outcomes.

At CoreMed Plus, technology is also an essential part of chronic disease management. The healthcare provider uses electronic health records (EHRs) to store patient information and facilitate communication between healthcare professionals. Patients can also use the provider’s patient portal to access their medical records, schedule appointments, and communicate with their healthcare team.

CoreMed Plus also recognizes the importance of addressing the social determinants of health in chronic disease management. Social determinants of health are the conditions in which people are born, grow, live, work, and age, and can significantly impact health outcomes. The healthcare team at CoreMed Plus works to identify and address these social determinants to improve patient outcomes and quality of life.

Chronic disease management at CoreMed Plus is focused on providing comprehensive, patient-centered care that addresses each patient’s unique needs. CoreMed Plus is committed to helping patients with chronic conditions manage their health effectively and improve their quality of life by offering education, personalized treatment plans, and a range of services and technologies.

What is a Chronic Disease

Chronic disease is a long-term medical condition that typically persists over a prolonged period, often for years or even for a person’s life. Chronic diseases can significantly impact a person’s quality of life, sometimes fatal if left untreated or unmanaged.

There are many different types of chronic diseases with varying causes and symptoms. Some of the most common chronic diseases include:

  1. Diabetes: Diabetes is a chronic disease when the body cannot produce or effectively use insulin, a hormone that regulates blood sugar levels. Over time, high blood sugar levels can cause damage to the kidneys, nerves, and other organs, increasing the risk of heart disease and other complications.
  2. Cardiovascular Disease: Cardiovascular disease refers to a range of conditions that affect the heart and blood vessels, including coronary artery disease, heart failure, and stroke. Risk factors for cardiovascular disease include high blood pressure, high cholesterol, smoking, and obesity.
  3. Chronic Obstructive Pulmonary Disease (COPD): COPD is a group of lung diseases that make breathing difficult, including chronic bronchitis and emphysema. COPD is most commonly caused by smoking and can cause significant disability and reduced quality of life.
  4. Cancer: Cancer is a group of diseases characterized by the uncontrolled growth and spread of abnormal cells. There are many different types of cancer, and risk factors can vary depending on the type of cancer.
  5. Arthritis: Arthritis is a group of conditions that cause joint inflammation, leading to pain, stiffness, and reduced mobility. There are many different types of arthritis, including osteoarthritis, rheumatoid arthritis, and gout.

These are just a few examples of the many chronic diseases that can affect people of all ages and backgrounds. While the causes and symptoms of chronic diseases vary widely, some common factors can increase the risk of developing a chronic disease.

Risk factors for chronic disease include:

  1. Genetics: Some chronic diseases, such as certain types of cancer, can run in families.
  2. Lifestyle Factors: Unhealthy lifestyle habits, such as smoking, poor diet, and lack of exercise, can increase the risk of developing chronic diseases.
  3. Environmental Factors: Exposure to certain environmental toxins or pollutants can increase the risk of developing chronic diseases.
  4. Age: As people age, the risk of developing chronic diseases increases.

While chronic diseases cannot always be prevented, there are many steps that people can take to reduce their risk and manage their symptoms. Lifestyle changes, such as quitting smoking, eating a healthy diet, and getting regular exercise, can help reduce the risk of developing chronic diseases. Regular health screenings can also help identify potential health issues early, when they are most treatable.

For people who are living with a chronic disease, managing symptoms and maintaining good health can be challenging. Treatment of chronic illnesses often involves a combination of medication, lifestyle changes, and support from healthcare providers and other caregivers.

At CoreMed Plus, we are committed to helping our patients manage chronic diseases and maintain optimal health and well-being. Our healthcare providers take a patient-centered approach to care, working with each patient to develop an individualized treatment plan that considers their unique needs and concerns. By working together, we can help our patients manage their chronic diseases and achieve their health goals.

What Can Cause a Chronic Disease

Chronic diseases are complex and often multifactorial conditions with many causes. In some cases, the cause of a chronic disease may be unknown, while in other cases, multiple factors may contribute to the development of the condition. Here are some common factors that can contribute to the development of chronic diseases:

  1. Genetics: In some cases, genetic factors can increase the risk of developing chronic diseases such as diabetes, heart disease, and certain types of cancer. Genetic mutations or abnormalities can lead to changes in the body’s processes, increasing the risk of developing these conditions.
  2. Lifestyle Factors: Unhealthy lifestyle habits, such as poor diet, lack of exercise, smoking, and excessive alcohol consumption, can significantly increase the risk of developing chronic diseases. For example, a diet high in saturated fats and processed foods can contribute to the development of heart disease, while smoking can increase the risk of lung cancer and other respiratory diseases.
  3. Environmental Factors: Exposure to certain environmental toxins or pollutants can also contribute to developing chronic diseases. For example, asbestos exposure can increase the risk of developing mesothelioma, while exposure to air pollution can increase the risk of respiratory diseases.
  4. Age: As people age, the risk of developing chronic diseases increases. Many chronic diseases, such as arthritis, osteoporosis, and Alzheimer’s, are more common in older adults.
  5. Chronic Inflammation: Chronic inflammation can contribute to the development of many chronic diseases, including heart disease, diabetes, and some types of cancer. Various factors, including poor diet, lack of exercise, and exposure to environmental toxins can cause inflammation.
  6. Infections: Certain infections can increase the risk of developing chronic diseases. For example, chronic hepatitis B or C infections can increase the risk of liver cancer and cirrhosis, while human papillomavirus (HPV) infections can increase the risk of cervical cancer.

It is important to note that chronic diseases are often complex and can have multiple causes. Some chronic conditions, such as type 1 diabetes, are caused by a combination of genetic and environmental factors. In contrast, others, such as heart disease, may be caused by various lifestyle factors and genetics.

Preventing chronic diseases often involves:

  • Making healthy lifestyle choices, such as eating a balanced diet.
  • Getting regular exercise.
  • Avoiding tobacco and excessive alcohol consumption.
  • Managing stress.

Regular health screenings can also help identify potential health issues early when they are most treatable.

    Dr. Mark Richter Bio

I grew up in Florida as the oldest of five sons. After earning my degree from the University of Florida, I pursued my medical education in the Dominican Republic. In 1988, I began my Family Practice Residency at the “old” Pontiac General Hospital. I have been providing medical care to the same community since 1991, first in Waterford and later at our current location in White Lake, where I continue to practice today. I take pride in being board certified by the American Academy of Family Physicians and remain committed to Primary Care. I am passionate about promoting a healthy lifestyle, including a proper “balanced” diet, exercise, exposure to healthy outdoor activities, and achieving work-life balance. I married my Medical School Sweetheart, Barbara, and we are fortunate to have two adult children and a granddaughter. We have been residents of Oakland County since 1988 and enjoy all that Michigan has to offer.
